Appointments
Doctor Lemagie sees patients in her office on Mondays, Wednesdays and Thursdays. Appointments can be made by calling 745-8379 between the hours of 8:30 am and 5 pm. Monday through Friday. If you would like a return phone call, please leave a message on the voicemail at the same number.
If you need to cancel an appointment please give us at least 24 hours notice so that we can offer that time to another patient.
New Patients are asked to fill out the forms provided on this page and send them in at least 24 hours prior to your first visit so that office staff can have time to confirm your insurance eligibility. When you arrive for your first visit please bring with you your insurance card(s) and a picture ID.
